On-line Forms

An On-Line form is one that can be completed while connected to the Internet and e-mailed directly to Dr. Makamson by clicking the "SUBMIT" button at the end of the form. You do not need to connect to your e-mail account for this to work as the form is e-mailed using the host web server.

A. Student Registration. This form is to be completed by all of Dr. Makamson's students. The information is used to establish each student's record for maintaining grades and provides the password code for posting of grades and for entry to the secured areas of this site.

B. Leadership Rating Form. For MGT 301 Only. This form is completed by a group member to evaluate another student's leadership ability.
